Double Up Food Bucks Site Report

• All sites must submit a separate report for each month. Submit a report for every month that the site was operating even if the site did not have any SNAP/Double Up activity.
• Passwords are no longer required to enter data. “Number of new customers” is no longer a required data point.
• This form is a one-way submission. If you need to edit data or would like to request a copy of your reports, please email [email protected]
• Farmers markets as well as all other site types will now report on a monthly basis. 
• Be sure to have your accompanying document(s) ready before you submit this form. Continue to upload those customer/vendor/EBT records to your dedicated Google Drive folder (refer to your Double Up contract for the link).
• Monthly reporting schedule: Reports from the prior month are due by the 10th day of the following month.
• All fields marked with an asterisk are required.
• Please note, this form contains conditional logic. Based on your responses, additional relevant questions may appear or disappear.
